We have a list of documents that you must submit to further your employment opportunity with Gentle Home Care, LLC.
The list of documents is as follows:
  • Driver’s License or State Id (front and back)
  • Social Security Card
  • Physical/letter from a physician within the last year
  • Covid-19 Vaccination Card
  • HHA/CNA/PCA/Homemaker certificate (if applicable)
  • BLS/CPR/First Aid
  • 2 professional references
  • Immunization Records (these can be your childhood immunizations or Titre) must include the following: You can obtain this information by going to https://myvaxrecords.mass.gov
  • TB within last year or Chest Xray within the last 3 years
  • Hep B within the last 2 years
  • Influenzas Immunization (Flu Shot) within the last year
  • Varicella Immunization (Chickenpox Shot)
  • Tdap (within the last 10 years)
  • MMR (titers/vaccination)

Once your application is completed, and all your documents have been submitted we will review your application, and credentials and schedule an orientation.
